Role Overview
A Clinical Documentation Improvement (CDI) Specialist reviews patient medical records to ensure completeness, accuracy, and clarity of clinical documentation, supporting proper coding, billing, and quality reporting. They act as a liaison between clinicians, coding teams, billing, and care coordination staff, promoting accurate capture of diagnoses, procedures, and treatments throughout a patient’s hospitalization or care episode.

Key Responsibilities
Review and analyze medical records to identify gaps, inconsistencies, or missing information in provider documentation.

Query providers for clarification on unclear, incomplete, or conflicting documentation to ensure accurate coding and reimbursement.

Collaborate with clinical and coding teams to improve documentation practices and support compliance with coding guidelines (ICD, CPT) and regulatory standards.

Monitor and manage CDI tracking tools and databases to validate DRG assignments, case mix index, and reimbursement accuracy.

Educate and advise providers on best practices for documentation, including outpatient and inpatient settings.

Support quality and safety initiatives, including severity of illness (SOI), risk of mortality (ROM), and core measure reporting.

Assist with denial management and ensure documentation supports accurate billing and risk adjustment.

Qualifications and Skills
Education: Typically requires a bachelor’s degree; many positions prefer a Registered Nurse (RN) or RHIA/RHIT certification.

Experience: Usually 4–7 years of clinical or coding experience, with advanced knowledge of inpatient coding and clinical care.

Skills: Strong analytical, communication, and interpersonal skills; ability to work independently and collaboratively; proficiency in medical terminology, coding guidelines, and electronic health records.

Work Environment
CDI Specialists work in hospitals, healthcare systems, or remotely, often reporting to a CDI manager.
